DOL Did Not Effectively Manage Mobile Devices and Related Telecommunication Services
The VA Office of Inspector General (OIG) investigated an allegation that the Executive Director of the Florida National Cemetery improperly stored his personal vehicle in a garage on Long Island National Cemetery property after he transferred to Florida and asked subordinates to drive him in government vehicles to and from his residence on the Long Island National Cemetery property to the airport. The Executive Director of the Calverton National Cemetery also allegedly asked subordinates to drive him in government vehicles to and from his residence and the airport. Additionally two employees allegedly misused VA resources by taking two government vehicles from New York to training in Virginia and one extended his travel to sightsee with his spouse. The OIG did not substantiate any of these allegations.
Tennessee Valley Authority business units are expected to find, analyze, and fix conditions that affect personnel safety, asset reliability, adverse trends, or other conditions that do not meet expectations. Condition reports (CR) are created to record how problems are found, analyzed, and solved. Due to the importance of finding, analyzing, and resolving concerns identified at coal plants, we conducted an evaluation of CRs at TVA coal plants to determine if (1) Coal Operations generated CRs for reported safety, environmental, and operational incidents; and (2) actions taken to address coal CRs were timely and effectiveWe found that CRs were originated for all environmental incidents; however, some safety and operational incidents did not result in CRs being originated as required by the procedure. We also found that not all actions taken to address CRs were timely, and some CR originators perceived that actions taken were not effective. In addition, we found opportunities for improvement related to (1) classification of CRs by significance level, (2) documentation of actions taken to address CRs, and (3) discrepancies in the Standard Programs and Processes that govern this process.
The OIG investigated allegations that a Bureau of Land Management (BLM) manager sent pornographic images and sexually suggestive instant messages from his Government computer and personal cellphone to a subordinate employee. The complainant further alleged that the BLM manager installed cameras in the district office to monitor employees and had misused Federal money to purchase a 72-inch flat-screen television and furniture for his office. We found that the BLM manager sent sexually explicit messages to a subordinate employee from his Government computer during work hours and to two other BLM subordinate employees from his personal cellphone. All involved said the exchange of content was consensual, but the manager acknowledged that the conduct was inappropriate.We did not find that the BLM manager installed cameras to monitor employees, but he did admit that he had inappropriately used the building’s surveillance system and his Government-issued iPad to capture images of BLM employees without their knowledge.We found no evidence that the BLM manager misused Federal money to buy a television or furniture for his office.
The Office of the Inspector General conducted a review of the Paradise Combined Cycle Plant (PCC) to identify operational and cultural strengths and risks that could impact PCC’s organizational effectiveness. We identified strengths that positively affected the day-to-day activities of PCC’s personnel and performance including, (1) organizational alignment, (2) teamwork, and (3) management support. However, we also identified issues that could pose risks to PCC’s effectiveness and its continued ability to meet its responsibilities, including achievement of Gas Operations’ initiatives in training and management communication.
OIG data analytics identified that Stockdale Station passport photo services revenue for October 1, 2017 to June 30, 2018 was $57,405 compared to $91,335 for October 1, 2016 to June 30, 2017, a 37 percent drop in revenue. For April through June 2018, the total revenue was $17,880 compared to $32,655 for the same period in 2017, a 45 percent drop in revenue. The objective was to determine whether passport photo revenue and fees were appropriately reported and whether opportunities for increasing photo services revenue were optimized at the Stockdale Station.
