Local Travel Reimbursements – Concord, CA, Main Post Office
OIG data analytics identified the Concord MPO recorded local travel reimbursement in Account Identifier Code (AIC) 538, Local Travel Transportation, totaling $26,834, or 79 percent of all local travel reimbursements in the Bay Valley District for the period April 1, 2018, through February 28, 2019. Local travel reimbursements at the Concord MPO were $9,805 at the end of the first quarter of fiscal year (FY) 2019 and $12,346 for two months of activity of Quarter (Q) 2, FY 2019, significantly higher than the prior quarters. The objective was to determine whether local travel reimbursements at the Concord MPO were appropriate and properly supported.
New York's Claims for Medicaid Nursing Home Transition and Diversion Waiver Program Services Generally Complied With Federal and State Requirements but Had Reimbursement Errors That Resulted in a Minimal Amount of Overpayments
During a prior review, we determined that New York claimed Medicaid reimbursement for home and community-based services (HCBS) under a Medicaid waiver program that did not comply with Federal requirements.New York's Nursing Home Transition and Diversion (NHTD) is an HCBS waiver program. Our objective was to determine whether New York claimed Medicaid reimbursement for NHTD waiver program services in accordance with certain Federal and State requirements.

Report Description
A new review conducted by the U.S. Army Corps of Engineers (the Corps) on behalf of SIGTARP found significant risks at the federal, state, and contractor level in the Hardest Hit Fund’s (HHF) $755 million Blight Elimination Program. These findings come nearly two years after SIGTARP warned the U.S. Department of Treasury that a similar review of demolitions in Flint, Michigan found program-wide risks of hazardous materials, illegal dumping, and contaminated soil.
Congress created the 340B Drug Pricing Program to assist providers serving vulnerable patient populations by allowing them to purchase outpatient drugs at discounted prices. For drugs dispensed to Medicare Part D beneficiaries for payment under the Part D program, manufacturers may pay rebates based on rebate agreement terms negotiated between the manufacturers and the Part D prescription drug plan sponsors or their pharmacy benefit managers. Rebate agreement terms may prevent manufacturers from paying rebates for Part D prescriptions filled at 340B contract pharmacies.
The Medicaid "health home" option allows States to create programs that provide care coordination and care management for beneficiaries with chronic health conditions. Health homes are not physical spaces. Rather, they are a health care model in which providers work together to coordinate and manage beneficiaries' care at a reasonable cost.
