Rec. 1.a: The DoD OIG recommend that the Deputy Assistant Secretary of the Army for Procurement reevaluate the contract actions with up to $828.5 million in unsupported costs, identified in Appendix B, to determine whether the price associated with the Ukraine security assistance was fair and reasonable and, based on the results of the review, take the necessary steps to recover any awarded cost that is more than the fair and reasonable price.

Rec. 2.a: The DoD OIG recommended that the Deputy Assistant Secretary of the Navy for Procurement Reevaluate the contract actions with up to $18.4 million in unsupported costs identified in the contract actions, listed in Appendix B, to determine whether the price associated with the Ukraine security assistance was fair and reasonable and, based on the results of the review, take the necessary steps to recover any awarded cost that is more than the fair and reasonable price.

Rec. 2.b: The DoD OIG recommended that the Deputy Assistant Secretary of the Navy for Procurement Issue a memorandum to remind Navy contracting officers of Federal Acquisition Regulation subpart 4.803, "Contents of Contract Files," and Federal Acquisition Regulation subpart 15.403, "Obtaining Certified Cost or Pricing Data," requirements to obtain all certified cost or pricing data to determine fair and reasonable prices and ensure that all data used to determine fair and reasonable prices are maintained as part of the contract file.

Rec. 3.a: The DoD OIG recommended that the Deputy Assistant Secretary of the Air Force for Acquisition, Technology, and Logistics Reevaluate the contract action with up to $0.7 million in unsupported costs, identified in Appendix B, to determine whether the price associated with the Ukraine security assistance was fair and reasonable and, based on the results of the review, take the necessary steps to recover any awarded cost that is more than the fair and reasonable price.

Rec. 3.b: The DoD OIG recommended that the Deputy Assistant Secretary of the Air Force for Acquisition, Technology, and Logistics issue a memorandum to remind Air Force contracting officers of Federal Acquisition Regulation subpart 4.803, "Contents of Contract Files," and Federal Acquisition Regulation subpart 15.403, "Obtaining Certified Cost or Pricing Data," requirements to obtain all certified cost or pricing data to determine fair and reasonable prices and ensure that all data used to determine fair and reasonable prices are maintained as part of the contract file.

We recommend the OFO San Diego Field Office Director, CBP, in coordination with partner agencies, regularly and continuously refine current strategies and identify new strategies to manage delays in detainee transfers to partners to meet time in custody standards at Calexico West POE.

Review and pay claimants the underpayments we identified, as applicable.

Add controls to ensure employees properly notify claimants when disability claims are denied and manual notifications are required.

Add controls to ensure employees complete all mandatory claim development prior to denying disability claims.

The principal executive director of the Office of Acquisition, Logistics, and Construction considers whether any additional training or other measures are necessary with respect to reporting the wrongdoing of a supervisor and the acceptance of free meals and drinks by VA employees during the February 2023 site visit.

The principal executive director of the Office of Acquisition, Logistics, and Construction determines whether any additional guidance, training, or oversight is needed with respect to ensuring VA employees do not improperly solicit sponsorships for VA events that do not primarily benefit veterans.

Establish and document physical security standards for the agency that include a risk-based decisionmaking framework to

b. document physical security decisions, including describing the rationale for any deviations from physical security standards.

Assign responsibility for managing third-party access cards for Board-leased spaces.

Ensure that the responsible group develops and implements a process to
a. collect and deactivate third-party access cards from offboarded Board personnel.
b. periodically reconcile third-party access card rights against human resources' list of active Board employees and contractors.

Establish a policy that defines the Technical Security Bureau’s responsibilities and the standard operating procedures needed to fulfill those responsibilities.
