Open Recommendations

We recommend that the Director of HUD's Buffalo Office of Community Planning and Development instruct City officials to Provide documentation to justify the $177,923 unsupported difference between the City's CDBG program income balance in IDIS and its bank account balances as of June 30, 2013. Any portion of the unsupported difference determined to be ineligible should be reimbursed from non-Federal funds.

The Bureau of Information Services should work with the Office of Administration, Division of Acquisition Management, and the agency’s contracting officer’s representatives to maintain the Contractor Security Control Log, ensure that it is continually updated to reflect all contractors and their staff that work at the RRB, and indicate whether the contractor staff will have access to personally identifiable information or a system of records.

The Bureau of Information Services should develop a strategic organizational privacy plan that is multi-organizational and represents the Railroad Retirement Board as a whole.

The Bureau of Information Services should develop a policy on communication between bureaus regarding changes in personally identifiable information.

The Bureau of Information Services should develop a policy on the use and protection of personally identifiable information in testing, training, and research.

Provide adequate supporting documentation for the $1,628,130 in unsupported salary and benefit costs or repay the CDBG program from non-Federal funds.

The Executive for Legislative Archives, Presidential Libraries, and Museum Services should Work with the Performance and Accountability Office to develop a performance measure for tracking the processing of electronic presidential records.

The Executive for Legislative Archives, Presidential Libraries, and Museum Services should determine the true backlog of electronic presidential records and determine if additional resources are needed and can be obtained to handle the increased workload.

Reimburse its HOME program $1,595,113 from non-Federal funds for HOME funds that were inappropriately used on Section 8 housing assistance payments.

Rec. A.2: The DoD OIG recommended that the Assistant Secretary of the Army (Financial Management and Comptroller) develop a working group, including the Assistant Chief of Staff for Installation Management, the Chief of the U.S. Army Corps of Engineers, and the General Fund Enterprise Business System Program Manager, to develop and implement the identified functionality into the General Fund Enterprise Business System, including the capability to generate an Army-wide real property universe.

Rec. A.6: The DoD OIG recommended that the Assistant Secretary of the Army (Financial Management and Comptroller) develop a working group, including the Assistant Chief of Staff for Installation Management, the Chief of the U.S. Army Corps of Engineers, and the General Fund Enterprise Business System Program Manager, to perform a review of all real property data in the General Fund Enterprise Business System to ensure that the General Fund Enterprise Business System contains the correct data going forward and track the costs associated with this effort and other data cleansing efforts so they can be calculated as part of…

Rec. A.7.b: The DoD OIG recommended that the Assistant Secretary of the Army (Financial Management and Comptroller) develop a working group, including the Assistant Chief of Staff for Installation Management, the Chief of the U.S. Army Corps of Engineers, and the General Fund Enterprise Business System Program Manager, to record in-house costs incurred in the construction of a real property asset to the corresponding project's construction-in-progress account.

(U) Rec. A.1: The DoD OIG recommended that the Office of the Under Secretary of Defense for Intelligence, partnering with the Defense Intelligence Agency, the Combatant Commands, and the Services, develop an All-Source Analysis certification program that leads to training, developing, and retaining a more experienced and robust workforce. We recommend that such a program includes common core analytical skills and performance standards, and an enterprise-wide all-source analysis occupational-specialty career track and development program.

We recommend that the Director of HUD's Detroit Office of Public Housing require the Commission to support or reimburse its program $425,193 ($19,924 $384,755 in housing assistance payments $46 in utility allowances $20,468 in associated administrative fees) from non-Federal funds for the unsupported overpayment of housing assistance and utility allowances due to unsupported calculations, missing eligibility documentations, and discrepancies in the housing assistance payments register.

The Chief Operating Officer should ensure a plan is developed, including a timeline, for when the archival storage facility reviews will be completed.  As a part of the reviews, identify facilities with (1) areas of non­-compliance, associated costs, risk if the actions are not completed, and an action plan, (2) structural, environmental control, fire safety, preservation, and security deficiencies that could be severe enough to permanently damage records.

The Chief Operating Officer should ensure an accurate listing of facilities currently non-compliant with the Standards, along with the area of deficiencies is identified and communicated.

The Chief Operating Officer should ensure resources needed to make all archival storage facilities compliant by 2016 are identified.  If the facility cannot be brought into conformance with the Standards, determine and document what mitigating actions have been or will be taken to minimize threats to the holdings.

The Chief Operating Officer should ensure PMRS is updated to accurately reflect percentage of archival holdings in appropriate space.

The Chief Innovation Officer and Executives for Research Services and Legislative Archives, Presidential Libraries and Museum Services should ensure comprehensive preservation policies and procedures for each of their organizations are developed and/or updated.

We recommend the Executive for Research Services should ensure An analysis is performed to determine if additional risk assessments for the Washington Area Archives and Presidential Libraries, including older holdings, should be completed.  Identify the risks for not completing the assessments.

Rec. A.1: The DoD OIG recommended that the U.S. Army Medical Command Commander revise Army Regulation 40-68, "Clinical Quality Management," to align the regulation with supervision requirements set forth in Federal Acquisition Regulation 37.4.

We recommend that FWS work with the Department to reconcile their respective records of lands purchased with Program funds.

We recommend that the Director of HUD's New York Office of Community Planning and Development instruct County officials to provide documentation to justify the $189,322 in unsupported administrative and planning costs that was disbursed for employee salaries and fringe benefits. Any unsupported costs determined to be ineligible should be reimbursed from non-Federal funds.

We recommend that the Director of HUD's New York Office of Community Planning and Development instruct County officials to reimburse from non-Federal funds $78,530 for ineligible home-buyer rehabilitation and demolition costs charged to the HOME program.

We recommend that the Director of HUD's New York Office of Community Planning and Development instruct County officials to terminate the contract between the County and the Village of Freeport to rehabilitate and construct single-family public housing units to be sold to low-income residents. The remaining contract balance of $31,470 should be put to better use by reprogramming it for other eligible purposes.
