Complete a cost analysis to 1) determine the efficacy of relocating CI assets in each of the field offices to existing IRS computer rooms versus upgrading the CI computer rooms to ensure that assets are protected in accordance with Federal and IRM security requirements and 2) implement the most cost-effective solution.

Determine if the newly standardized LDR data are reliable for direct maintenance workloads in the Technical Operations Staffing Model, and if so, develop and implement an action plan with milestones to replace the workload assessments with LDR data.

We recommend that the State agency • strengthen its controls over its process for reporting expenditures claimed for NEMT services.

We recommend that the State agency • improve its oversight and monitoring of its Medicaid NEMT brokerage program by requiring LogistiCare to strengthen its procedures to ensure that (1) NEMT services are adequately documented and the documentation maintained according to Federal and State regulations; (2) transportation provider qualifications meet State requirements; and (3) vehicle inspection, safety, and insurance requirements are met.

We recommend that the State agency • ensure that the State agency’s contract with the transportation broker contains provisions that (1) consider improper claims submitted by transportation providers to the transportation broker when developing future capitated rates paid by the State agency and (2) provide a means for the State agency to recoup funds from the transportation broker when contract provisions and State requirements are not met—a measure that, if incorporated, could result in cost savings for the Medicaid program.

Clearly define all the roles to be used in the agency’s OEP.

Develop and implement an effective OEP team training program with drills and exercises to include all team members at least annually.

Develop and implement procedures to address the needs of individuals requiring additional assistance. These procedures should include a process to routinely update the list of persons requiring assistance.

Develop and implement procedures to maintain, retain, and update OEP program documents at least semi-annually.

Develop and implement an annual round-table discussion with OEP coordinators and teams.

Develop and implement facility-specific policies and procedures.

Rec. 1.b.i: The DoD OIG recommended that the Assistant Secretary of Defense for Energy, Installations, and Environment issue guidance to identify the roles, responsibilities, and deciding officials for key segments of a facility construction project, including but not limited to, the project development, budgetary submissions, design reviews, planning, construction management, and assessment of contractor performance.

Rec. 1.b.ii: The DoD OIG recommended that the Assistant Secretary of Defense for Energy, Installations, and Environment issue guidance to establish metrics that include financial risk management parameters and triggers, including, but not limited to, threshold changes to scope, cost, or timeline; emerging issues; dispute resolution; and statutory reporting requirements when higher headquarters engagement is required.

Rec. 3.a: The DoD OIG recommended that the U.S. Army Corps of Engineers Commander review the actions of the individuals involved in the Fort Bliss Hospital Replacement project to determine whether any actions resulted in the cost and time increase related to design errors and omissions

Rec. 3.b: The DoD OIG recommended that the U.S. Army Corps of Engineers Commander issue guidance to improve technical expertise and discipline for medical infrastructure projects and improve understanding of performance specifications and extensions of design and performance metrics for projecting a project at risk.
