Develop and implement formal processes and procedures to ensure allegations of egregious harassment raised in EEOcomplaints can be referred to OPR for separate, potential misconduct review at the discretion of the OER director.

(U) Rec. 1.a: The DoD OIG recommended that the Under Secretary of Defense for Acquisition and Sustainment develop and issue implementing guidance for DoD supply chain risk management for DoD materiel in accordance with DoD Instruction 4140.01.

(U) Rec. 2.b.2: The DoD OIG recommended that the Director of the Defense Health Agency complete the implementation of its Drug Supply Chain Security Act Compliance Strategy by, in coordination with the Commander of Defense Logistics Agency Troop Support, establishing policy and procedures for drugs purchased with Defense Logistics Agency-managed processes, including Defense Logistics Agency Depot Stock, Direct Vendor Delivery, and Defense Logistics Agency Electronic Catalog.

Update Customs Directive No. 4320-003, July 1990 (TECS Directive) to clarify the appropriate bases for placing lookouts and provide training to all CBP officials who have the authority to place lookouts.

Develop and implement procedures to ensure CBP officials update and remove lookouts in accordance with the TECS Directive.

Develop and issue a policy regarding asking, advising, or otherwise communicating with foreign governments about denying entry to U.S. citizens. At a minimum, the policy should specify the appropriate circumstances for such communications, who is authorized to approve such communications, and the procedures to follow when making such communications.

CMS should share information to help States address challenges with using telehealth to provide behavioral health services to Medicaid enrollees

CMS should conduct monitoring for fraud, waste, and abuse, and support State efforts to oversee telehealth for behavioral health services.

CMS should ensure that the three States that are unable to distinguish telehealth from in-person services implement indicators to identify which services are provided via telehealth.

CMS should conduct evaluations, and support State efforts to evaluate the effects of telehealth on access, cost, and quality of behavioral health services.

Develop and implement an action plan to reduce turnover of management and staff and to ensure continuity of Unit operations should turnover occur

The bureau should award Mission Iraq-specific task orders under the Diplomatic Platform Support Services contract or use an alternative mechanism to competitively replace the Operations and Maintenance Support Services and Bagdad Life Support Services contracts.

OIG recommends that the Bureau of Administration, Office of the Procurement Executive (OPE), strengthen its review and approval process and oversight of noncompetitive contract actions awarded, based on Federal Acquisition Regulation 6.302-2, “Unusual and compelling urgency,” related to circumstances permitting other than full and open competition. OPE should put in place controls to ensure those actions are not excessive, such as enforcement of time limitations on the duration of the use of these actions, and use appropriate rationales, which exclude lack of advanced planning.
