We recommend that USAID's Chief of Travel and Transportation revise the system security plan to require account managers for the End-to-End Travel system to be notified about personnel separations in a timely manner and receive sufficient time to disable system access within 24 hours, rather than 3 days, of a user's separation.

CMS should obtain the Federal shares of MLR remittances from States that failed to return these amounts to CMS for the 2017-2018 and 2018-2019 MLR reporting years.

CMS should develop the capacity to systematically track and readily determine the amounts of the Federal shares of MLR remittances that States return to CMS.

CMS should improve internal communication to confirm that States return the Federal shares of the MLR remittance amounts reported annually to CMS.

We recommend that the General Counsel ensure all nondisclosure agreements and forms currently in use by the
Department and identified in this report as noncompliant with the WPEA are updated to include the anti-gag provision statement.

We recommend that the General Counsel develop and issue written policies and procedures and create a process, to include identifying those responsible for the implementation and management of the process, to ensure that all nondisclosure forms and agreements, including settlement agreements, are tracked and comply with Federal law and include the anti-gag provision statement when required.

We recommend that the Acting Assistant Administrator for USAID's Bureau for Latin America and the Caribbean document management controls for U.S. foreign assistance to Colombia such as by defining key terms, inserting specific appropriations law language in activity-specific statutory checklists for any new obligations, and clarifying how limitations apply to unspent prior year funds.

We recommend that the Acting Assistant Administrator for USAID's Bureau for Latin America and the Caribbean formalize guidance for implementing USAID's more restrictive stance of not providing assistance that targets demobilized combatants.

Define in policy
a. the process that examiners should use to request access to files in the Supervision Examination System.
b. the criteria that managers and regional analysts should use to assess whether a need to know exists in accordance with the least privilege principle.
c. the requirement that regional analysts document an examiner’s need to know before granting access to supervision files in the Supervision Examination System.
d. consequences for accessing confidential supervisory information without a need to know or providing access to confidential supervisory information when a need to know does not exist.

Develop and require training for CFPB staff involved in the examination process for the policy and guidance resulting from recommendations 1 and 2.

Update the guidance for managing breaches of confidential supervisory information to include expectations for
a. assessing and documenting the level of harm associated with a breach.
b. counseling, training, or taking other measures to hold CFPB staff responsible for breaches accountable, as appropriate, and documenting such actions.
c. analyzing the causes of breaches to identify trends and implement appropriate control adjustments, as necessary.

Develop required training on the updated guidance after it is implemented.

Update the CFPB’s confidential information breach response directive to
a. provide guidance for assessing the risk to institutions affected by breaches of confidential supervisory information and notifying those institutions.
b. define the roles and responsibilities for those involved in the process.

Rec. 1.a: The DoD OIG recommended that the Under Secretary of Defense for Personnel and Readiness revise and reissue policy requiring the Military Departments to follow a uniform process to identify, notify, and report child abuse allegations to parents or legal guardians across all DoD-sanctioned activities, including child development centers.
