Identify, review, and approve a CHDO to work with the City to ensure that $254,215 in CHDO funds will be properly committed to avoid being deobligated.

Establish underwriting policies and procedures to ensure that HOME activities are consistent and meet Federal requirements.

Develop and implement adequate environmental policies and procedures to ensure that environmental reviews are properly documented and supported and that HUD and Federal environmental requirements have been followed before committing HOME funds to an activity.

Develop and implement adequate policies and procedures to ensure that fixed HOME units are identified and adequate documentation is maintained to support tenant eligibility and compliance with HOME rental limits for the entire affordability period.

We recommend that the Office of the Director ensure that the CIO has adequate involvement and approval in all phases of annual and multi-year planning, programming, budgeting, and execution decisions in line with FITARA and OMB Circular A-130 requirements.

We recommend the Bureau of Fiscal Operations develop or update, and implement comprehensive written policies and step by step procedures for all write-off and waiver processes, to include day to day operations, documentation and approvals, roles and responsibilities, and threshold requirements, to ensure consistency and compliance with applicable laws, regulations, and guidance.

We recommend the Bureau of Fiscal Operations develop and implement controls to ensure debt balances that can be collected are not forfeited due to automatic $25 write-off or $100 waiver thresholds.

We recommend the Bureau of Fiscal Operations conduct training on new policies and procedures to ensure that decision making is consistent.

We recommend the Bureau of Fiscal Operations update Management Control Review documentation to reflect the new policies and procedures and changes in write-off and waiver processing which resulted from the migration to Financial Management Integrated System.

Embassy Bogota should remove the KACTUS database from the dedicated internet network and conduct a risk assessment based on the National Institute of Standards and Technology’s Risk Management Framework.

Rec. B.1.e: The DoD OIG recommended that the Secretaries of the Army, Navy, and Air Force take prompt action to ensure that the importance of complying with DoD and supplemental Military Service policies related to law enforcement's response to domestic violence incidents when collecting evidence, conducting interviews, notifying Family Advocacy Program staff members, and titling and indexing subjects in the Defense Central Index of Investigations is emphasized in writing to all law enforcement organizations.

Rec. B.1.f: The DoD OIG recommended that the Secretaries of the Army, Navy, and Air Force take prompt action to ensure that law enforcement practices, equipment, and supervisory reviews are adequate to comply with DoD policies when collecting evidence, conducting interviews, notifying Family Advocacy Program staff members, and titling and indexing subjects in the Defense Central Index of Investigations.

Rec. 1.b: The DoD OIG recommended that in accordance with Public Law 115-232, Section 2862, paragraphs a through c, the Under Secretaries of Defense for Personnel and Readiness and for Acquisition and Sustainment, in coordination with the Services, develop and implement a plan to field and sustain DoD-wide solutions to address training gaps, including addressing (1) the airspace and impact area needs of advanced aircraft and weapons, such as the F-35; and (2) the need to join neighboring airspace on a continuing basis.

Rec. 2: The DoD OIG recommended that the Under Secretary of Defense for Personnel and Readiness, in coordination with the Army and the Air Force, develop and implement plans to synchronize Army and Air Force range management and range use in Alaska for: a) joint training events, b) individual through collective level training for the Army and the Air Force, and c) future F-35 training needs across the DoD to ensure readiness and the ability to accomplish operation plans.

Design controls to protect the insurance fund from improper partial claims that did not reinstate the loans to put $27.1 million to better use.
