A.1.a: The DoD OIG recommended that the Defense Health Agency Director coordinate with Brooke Army Medical Center and Landstuhl Regional Medical Center management, through U.S. Army Medical Command, to develop a plan to review the delinquent debt identified in the prior audit reports, Report No. DODIG-2014-101 and DODIG-2016-079.

USAID/Zimbabwe determine the allowability of $648,515 in questioned costs ($277,171 ineligible, $371,344 unsupported) identified on page 15 to 18 of the audit report and recover any amount that is unallowable.

We recommend that the Secretary of the Interior use discretionary authority to continue or eliminate the maintenance fee waiver for small miners based on the results of the analysis performed by BLM.

We recommend that the State agency review all claims with dates of admission before our audit period (from July 1, 2012, and paid through December 31, 2012), and all claims paid after our audit period (June 30, 2017), to determine whether payments should be reduced for any claims that contained PPCs and refund to the Federal Government its share of any unallowable amounts.

Clarify program responsibilities between the Veterans Health Administration and theOffice of Operations, Security, and Preparedness, and evaluate the need for a centralizedmanagement entity for the security and law enforcement program across all medicalfacilities.
