Open Recommendations

We recommend that the PBS Regional Commissioner of the Pacific Rim Region take immediate action to improve management oversight of the region’s facilities to ensure that: (1) code violations and maintenance deficiencies brought to management’s attention are immediately addressed; (2) local fire department personnel are involved in pre-fire planning activities for future battery system projects; (3) local fire department personnel are involved in post-fire assessments; and (4) final certificates of occupancy are not issued until all outstanding fire and life safety deficiencies are corrected.

Rec. A.1: The DoD OIG recommended that the Under Secretary of Defense for Policy update guidance and identify specific steps to address highly sensitive information requests that require additional coordination and approvals, including the levels of coordination required to efficiently transfer information to facilitate the Government Accountability Office engagement, in accordance with 31 U.S.C. 716, and for a more transparent process for tracking requests.

Rec. A.2.a: The DoD OIG recommended that the DoD Performance Improvement Officer and Director of Administration and Management establish and consistently implement guidance and training for all primary action officers, including their roles and responsibilities, as well as procedures for processing Government Accountability Office information requests, including requests for highly sensitive information, in accordance with 31 U.S.C. 716.

Rec. A.2.b: The DoD OIG recommended that the DoD Performance Improvement Officer and Director of Administration and Management engage with the Government Accountability Office to develop and implement mutually agreed?upon processes and procedures for the DoD Audit Management Division and the Government Accountability Office during engagements that involve highly sensitive information, including the roles of the DoD Audit Management Division liaisons and primary action officers.

(U) Rec.1: The DoD OIG recommended that the Assistant Secretary of Defense for Acquisition, update existing intellectual property policy and include a responsibility for program management offices to maintain documentation that supports the defined and validated data requirements.

(U) Rec 2: The DoD OIG recommended that the Assistant Secretary of the Air Force for Acquisition, Technology, and Logistics, require the following program management office personnel supporting major weapon system acquisitions to complete and maintain the Defense Acquisition University's Foundational Intellectual Property Credential. a. (U) Program Manager. b. (U) Deputy Program Manager.c. (U) Contracting Officer. d. (U) Data Manager or equivalent.

We recommend the Council document and implement FFR reporting procedures that adhere to cost allowability restrictions prohibiting state agencies from reporting subrecipient cost shares as their own cost share (General Terms #13), or reporting the same costs across multiple awards (2 CFR 200.403).

We recommend the Council establish controls over its FFR reporting procedures that ensure the procedures were followed and report costs are allowable.

We recommend the Council works with its subrecipient to ensure understanding of, and compliance with, Poetry Out Loud (POL) regulations and requirements.

We recommend the Council update its contract template to include the total amount of that Arts Endowment award's funds issued to the subrecipient to date, and the total amount of Arts Endowment funds issued to the subrecipient from all active Arts Endowment awards (2 CFR 200.332(b)(1)(viii) & (ix)).

We recommend the Council document subrecipient notification procedures that adhere to the requirements established in 2 CFR 200.332(b)1, and establish controls that ensure the procedures are followed.

We recommend the NEA disallow $5,894,428 of unallowable subrecipient cost share from the 2019 Award.

We recommend the NEA disallow $6,566,008 of duplicate costs - $5,673 from the 2020 Award and $892,400 from the 2021 Award.

We recommend the NEA disallow $16,175 in unsupported costs - $500 from the 2019 Award and $15,675 from the 2021 Award.

We recommend the NEA review any additional Poetry Out Loud costs provided by the Council and determine allowability.

We recommend the NEA disallow $7,206 in unallowable Poetry Out Loud cash prize and reception costs - $3,600 from the 2020 Award and $3,606 from the 2021 Award.

We recommend the NEA disallow $3,154 in pre-award subrecipient costs from the 2019 Award.

We recommend the NEA disallow $503 in unreasonable subrecipient costs from the 2019 Award.

Verify that Palestinian Peace Coalition corrects the two material weaknesses in internal control detailed on pages 22 and 23 of the audit report.

(U) Rec. 1: The DoD OIG recommended that the Commanding General, Army Materiel Command, develop and implement a more effective process to review and provide written confirmation of a dormant stock item review on an annual basis.

(U) Rec. 2.a: The DoD OIG recommended that the Commanding General, U.S. Army Tank-Automotive and Armaments Command conduct a cost-benefit analysis to determine whether it should keep the M88 spare parts with no demand in the last 2 years for future use or dispose of the unnecessary spare parts.

The Under Secretary for Health ensures the establishment of Veterans Integrated Service Network-level multidisciplinary cancer committees.

The Under Secretary for Health ensures Veterans Integrated Service Network staff submit an inventory of available oncology services and facility points of contact to the National Oncology Program Office annually.

The Under Secretary for Health ensures complexity level 1 and 2 facilities pursue membership in the National Cancer Institute’s National Clinical Trial Network or the National Cancer Institute Community Oncology Research Program.
