Develop a comprehensive process to ensure that complaints received by HUD's Multifamily Housing Clearinghouse are resolved in a timely manner. Status

Develop agencywide policies and procedures for the intake, monitoring, and tracking of health and safety complaints. Status

Develop an automated real-time system for HUD and PBCA staff to use to receive, track, and resolve health and safety issues.

Revise the annual contributions contract to more clearly define the required treatment of health and safety issues, to include: Specific timeliness requirements for resolving life-threatening and non-life-threatening health and safety issues. Notification that HUD will actively monitor the status of complaints and become involved in resolution if necessary. Requirement that PBCAs will immediately contact HUD staff if a property has a life-threatening or non-life-threatening health and safety issue and report when the issue is resolved.

Issue Tier 1 test orders for each List 2 chemical or publish an explanationfor public comment on why Tier 1 data are no longer needed to characterize a List 2 chemical’s endocrine-disruption activity.

Determine whether the EPA should incorporate the Endocrine Disruptor Screening Program Tier 1 tests (or approved new approach methodologies) into the pesticide registration process as mandatory data requirements under 40 C.F.R. § 158 for all pesticide use patterns.

Issue List 1–Tier 2 test orders for the 18 pesticides in which additional Tier 2 testing was recommended or publish an explanation for public comment on why Tier 2 data are no longer needed to characterize the endocrine-disruption activity for each of these 18 pesticides.

Issue for public review and comment both the Environmental Fate and Effects Division’s approach for the reevaluation of List 1–Tier 1 data andthe revised List 1–Tier 2 wildlife recommendations.

Develop and implement an updated formal strategic planningdocument, such as the Comprehensive Management Plan.

Develop performance measures, with reasonable time frames, to document progress toward and achievement of milestones or targets. Specifically, the Endocrine Disruptor Screening Program should consider at least one performance measure that tracks progress in testing pesticides for human endocrine disruptor activity.

Conduct annual internal program reviews of the Endocrine DisruptorScreening Program.

Complete and publish the Endocrine Disruptor Screening Program’s response(s) to 2015 Federal Register notice comments and its relatedwhite paper.

To increase external communication and transparency, update theEndocrine Disruptor Screening Program website, including the program timeline, and publish any relevant program documents.

We recommend that the Arkansas Department of Human Services establish policies and procedures to track its private MMIS contractor costs to APDs and to ensure that it adheres to the funding and time-period limits established in those APDs.

Simplify the EEOC’s social media architecture by consolidating many of the existing social media channels and limiting creation of new channels

. Implement a consistent content approval process run by OCLA

Implement an ongoing training series for all staff involved in the EEOC social media program.

Use paid media to support improved reach and engagement.

Complete revisions to the social media handbook and provide to all staff managing social media channels.

Establish and implement a process to ensure that The Office of Multifamily Housing Programs' policies, procedures, and supervisory controls are effective. This process should include addressing postdisaster damage assessments, properly updating iREMS, and executing loan forbearances. This process should also integrate with other HUD program offices as appropriate to improve consistency with HUD's overall disaster response and to ensure the effectiveness of disaster controls.

OIG recommends that the Bureau of Administration, Office of the Procurement Executive amend the Foreign Affairs Handbook to include a subsection in 14 FAH-2 H-320, Acquisition Planning, providing direct guidance on sole source noncompetitive contracts or contract extensions with an existing contractor to bridge the time between the original end of that contractor’s contract and the competitive award of a follow-on contract, including providing definitions to the terms: “bridge contract” or “bridge actions,” the parameters under which these sole source noncompetitive contracts and extensions can be used, and a requirement to provide additional information necessary to justify consecutive uses of…

OIG recommends that the Bureau of Administration, Office of the Procurement Executive, develop and implement a process to identify and elevate to the Assistant Secretary of Administration, who serves as the appointed agency Chief Acquisition Officer, instances when the acquisition planning process is adversely impacted due to insufficient program office participation so the Chief Acquisition Officer can engage with the impacted program office bureau’s assistant secretary to resolve delays in pursuing full and open competition for contracts in overseas contingency environments.

OIG recommends that the Bureau of Administration, Office of the Procurement Executive, revise its Justification for Other than Full and Open Competition (JOFOC) Guide to require that written justification for using less than full and open competition for awarding bridge contracts include information on why and how the delay occurred, what measures have or will be undertaken to resolve the delay, and the milestones to achieving full and open competition.

Rec. A.1.b: The DoD OIG recommended that the Under Secretary of Defense for Acquisition and Sustainment revise DoD Instruction 4715.18 to include requirements for Emerging Chemical Program officials to develop risk management options and initiate proactive risk management actions which may be warranted to identify and mitigate the contaminant effects of emerging chemicals as early as possible in the Emerging Chemical Process, regardless of whether an emerging chemical is on the Emerging Chemical Watch List or the Emerging Chemical Action List.

Rec. A.2: The DoD OIG recommended that the Deputy Assistant Secretary of Defense (Environment and Energy Resilience) complete the Emerging Chemical Process for potential perfluoroalkyl and polyfluoroalkyl substances exposure caused by DoD activities from perfluoroalkyl and polyfluoroalkyl substance-containing materials other than Aqueous Film Forming Foam by developing and presenting validated risk management options for the perfluoroalkyl and polyfluoroalkyl substances on the Emerging Chemical Action List to the Emerging Chemicals of Concern Governance Council, as required by DoD Instruction 4715.18.
