Open Recommendations

We recommend that the Office of Wildland Fire develop and ensure implementation of a Departmentwide policy to standardize inspection and preparedness review requirements for wildland fire vehicles, which should include, at a minimum, inspection intervals and documentation requirements.

We recommend that the Office of Wildland Fire develop and implement a Departmentwide oversight plan, including compliance monitoring and specific corrective actions, to ensure vehicle inspections are completed in accordance with the policy created in Recommendation 1.

We recommend that the BIA conduct inspections of all active wildland fire vehicles and complete preparedness reviews in accordance with policy.

The Under Secretary for Health ensures legacy site providers are aware of mitigations needed for patients previously treated at a new electronic health record site and monitors compliance.

The Under Secretary for Health ensures that pharmacist staffing levels are assessed and addressed prior to the implementation of the new electronic health record at additional VA sites.

The Under Secretary for Health evaluates the underlying technical and functional issues resulting in workarounds and educational materials needed to perform pharmacy-related operations within the new electronic health record and takes action as indicated.

We recommend that the Assistant Secretary for Elementary and Secondary Education require MDE to update its policiesand procedures to reflect that all changes to Mississippi’s approved State plan should be submitted to the Departmentand approved by the Department before implementing changes to the accountability system.

We recommend that the Assistant Secretary for Elementary and Secondary Education require MDE to provide CSI to thefour public schools that should have been identified for CSI in the fall of 2022.

We recommend that the Assistant Secretary for Elementary and Secondary Education require MDE to submit anamendment explaining that it will apply double weighting to the numerator for students who took and passed certainaccelerated courses and describing how it will calculate the college and career readiness indicator score if a studenttakes both the assessment designed to measure skills that are important for success in postsecondary education andthe assessment designed to measure foundational skills required for success in the workplace.

We recommend that the Assistant Secretary for Elementary and Secondary Education require MDE to provide records tothe Department showing that it corrected the coding in the MSIS to ensure that the system uses unadjusted compositescores to identify nontraditional public high schools for CSI.

We recommend that the Assistant Secretary for Elementary and Secondary Education require MDE to provide records tothe Department showing that it ensured that all Title I, Part A-funded schools with 3 years of accountability data areincluded on the list that MDE uses for identifying public schools that are eligible for CSI.

(U) Rec. 1.a: The DoD OIG recommended that the Commander of the 379th Air Expeditionary Wing develop and implement procedures requiring incoming 379th Air Expeditionary Wing weapons safety managers to Review all in-process and completed explosives safety site plans to ensure explosives safety site plans are current.

(U) Rec. 1.b: The DoD OIG recommended that the Commander of the 379th Air Expeditionary Wing develop and implement procedures requiring incoming 379th Air Expeditionary Wing weapons safety managers to review net explosive weight limits in the Theater Integrated Combat Munitions System for all munitions storage structures to ensure they align with net explosive weight limits in DoD Explosive Safety Board-approved explosives safety site plans.

(U) Rec. 1.c: The DoD OIG recommended that the Commander of the 379th Air Expeditionary Wing develop and implement procedures requiring incoming 379th Air Expeditionary Wing weapons safety managers to Verify at least quarterly the net explosive weight totals in all munitions storage structures at Al Udeid Air Base to ensure all organizations storing munitions comply with net explosive weight limits.

(U) Rec. 2.a: The DoD OIG recommended that the Commander of U.S. Air Forces Central Detachment 6 ensure contractor personnel update courtesy storage agreements to require tenant organizations to initially report all munitions data, including net explosive weight totals, and to report changes in munitions storage to the contractor.

(U) Rec. 2.b: The DoD OIG recommended that the Commander of U.S. Air Forces Central Detachment 6 ensure contractor personnel track the net explosive weight totals for all munitions stored in the munitions storage area.

(U) Rec. 2.c: The DoD OIG recommended that the Commander of U.S. Air Forces Central Detachment 6 ensure that the contracting officer's representative audits include the contractor's compliance with tracking net explosive weight totals for all munitions, including tenant organization munitions, stored in the munitions storage area.

As expeditiously as possible, Treasury's Office of the Assistant General Counsel for General Law, Ethics and Regulation (GLER) should develop and implement standard operating procedures, including monitoring, to ensure ethics officials fully utilize the Ethics Tracker database to track requests, substantive advice and counsel provided, as well as documentation of final resolution of issues.

As expeditiously as possible, Treasury's Office of the Assistant General Counsel for General Law, Ethics and Regulation (GLER) should consider completing a cost-benefit analysis to determine if a fully integrated and automated tracking system would benefit GLER with its ethics responsibilities.
