The Commissioner, LB&I Division, should develop procedures to ensure that all large partnership returns filed throughout the year are considered for risk assessment by the LPC Program.

We recommend that the Millennium Challenge Corporation determine the allowability of $27,000 in ineligible questioned costs on pages 40 and 41 of the audit report and recover any amount that is unallowable.

Determine whether the more than $83.3 million charged in FY 2024 for workstation services to customers were supported and allowable, and recover as appropriate.

Establish control activities, in accordance with GAO’s Green Book, to include procedures for workstation services billing to ensure bills submitted to agencies are accurate and supported.

Update policies and procedures to comply with the 2007 Code of Federal Regulations and the 2015 USDA Agriculture Property Management Regulations as required, to ensure that computers with memory capabilities are classified as sensitive personal property.

Rec. 1: The DoD OIG recommended that the Commander, Naval Supply Systems Command, review the four national stock numbers identified in this audit for which Navy personnel could not provide evidence of full restitution. If the review determines that the contractor did not provide restitution and the Navy can pursue restitution under the contract terms, then the Commander should require the applicable Navy contracting officer to pursue restitution from the contractor that provided defective parts. Rec. 2.a: The DoD OIG recommended that the Deputy Assistant Secretary of Defense for Logistics update Defense Logistics Management Standards DLM 4000.25, Volume 2, Chapter 24 Product Quality Deficiency Reporting, to require Military Departments to develop a process for obtaining restitution from contractors that provide defective parts. The guidance should clearly define key roles and responsibilities of responsible contracting officers and require their involvement in the process and require the Military Departments to:preserve an audit trail to support the receipt of restitution and specify the type of documentation needed, andestablish a program to provide recurring training to deficiency reporting personnel…

Rec. 2.b: The DoD OIG recommended that the Deputy Assistant Secretary of Defense for Logistics require Military Departments to develop and implement controls and oversight to ensure tracking of contractor restitution from the completion of the investigation of the report on deficiencies in product quality through the receipt of restitution.

(U) Rec. 1.a: The DoD OIG recommended that the Director of the Defense Health Agency conduct an assessment of the physical security of all National Capital Region military treatment facilities, with a focus on identifying deficiencies with Unified Facilities Criteria 4-510-01 requirements.

(U) Rec. 1.b: The DoD OIG recommended that the Director of the Defense Health Agency develop a plan with milestones based on this assessment to:(U) provide the necessary resources for the military treatment facilities to comply with Unified Facilities Criteria 4-510-01.(U) ensure that the Andrews Medical Group - Malcolm Grow Medical Clinics and Surgery Centers video surveillance system meets minimum Unified Facilities Criteria 4-510-01 standards.

(U) Rec. 1.c: The DoD OIG recommended that the Director of the Defense Health Agency provide Program Objective Memorandum guidance to address the Walter Reed National Military Medical Centers video surveillance system deficiencies.

(U) Rec. 1.d: The DoD OIG recommended that the Director of the Defense Health Agency develop and implement a policy to standardize emergency response procedures across all military treatment facilities to ensure military treatment facilities have standardized emergency response procedures that bridge the existing gap in emergency response capabilities between military treatment facilities and their host installations. At a minimum, this policy should:(U) provide comprehensive training requirements for military installation police, on-site security guards, and staff, as well as a requirement to have regular drills and training exercises(U) establish an emergency response window for on-site guards and military police that…

(U) Rec. 2.a: The DoD OIG recommended that the Commander of the Navy Installations Command conduct a thorough evaluation of how the Naval Support Activity Bethesda police are used at Walter Reed National Military Medical Center, including how the Walter Reed National Military Medical Centers security needs are addressed.

(U) Rec. 2.b: The DoD OIG recommended that the Commander of the Navy Installations Command develop and implement a plan of actions with milestones to address any gaps identified in the evaluation.

We recommend that the Assistant Secretary for Elementary and Secondary Education require Puerto Rico DOE to return the $3.9 million charged to the ARP ESSER grant for the student academic proficiency interim assessments or take other remedial actions as appropriate, such as making accounting adjustments to other valid and allowable obligations incurred during the ARP ESSER grant period of availability.

We recommend that the Assistant Secretary for Elementary and Secondary Education require Puerto Rico DOE to develop and incorporate into its existing contract monitoring and oversight policies and procedures, control activities, or protocols designed to ensure that only qualified personnel are assigned to manage and oversee contracts and those charged with that responsibility verify that the contractually required services are rendered (for example, by reviewing documentation supporting the completion of contract-related work) before paying the contractor.

We recommend that the Assistant Secretary for Elementary and Secondary Education require Puerto Rico DOE to implement its contract amendment procedures, as designed, to ensure that all future changes to the scope of contracted work are documented and approved in writing.
