Report Description:
This Office of Inspector General (OIG) Comprehensive Healthcare Inspection Program report describes the results of a focused evaluation of the leadership performance and oversight by Veterans Integrated Service Network (VISN) 4: VA Healthcare in Pittsburgh, Pennsylvania.
This evaluation focused on five key areas of clinical and administrative operations:
• Leadership and organizational risks
• Quality, safety, and value
• Medical staff credentialing and privileging
• Environment of care
• Mental health (focusing on suicide prevention)
The OIG issued two recommendations for improvement in two areas:
• Medical Staff Credentialing and Privileging
o Credentials files review and appointment recommendations for physicians with a history of licensure action
• Environment of Care
o Annual Comprehensive Environment of Care compliance report submission to the Environment of Care Committee
